The National Insurance Office is appealing to all those employers who received correspondence entitled Notice of Appearance by Respondent (Form 5) in respect of former employees, to complete and return these forms to the National Insurance Office.

These forms should be placed in the drop-boxes at the Frank Walcott Building no later than Friday, March 19.

The National Insurance Office said that no scanned or email copies will be accepted.
